Live Well Wellness Center – Your Information. Your Rights. Our Responsibilities.
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED, HOW YOU CAN ACCESS THAT INFORMATION, AND HOW TO FILE A PRIVACY COMPLAINT. PLEASE REVIEW IT CAREFULLY.
Effective Date: October 14, 2026
1. Introduction
At Live Well Wellness Center, we are committed to protecting the privacy and confidentiality of our patients’ medical information.
This Notice of Privacy Practices explains how Live Well Wellness Center (“we,” “us,” “our,” or “the Practice”) may use and disclose your protected health information (PHI), your rights regarding that information, and our responsibilities under the Health Insurance Portability and Accountability Act of 1996 (HIPAA), its implementing regulations, and applicable Arizona law.
This notice applies to protected health information created, received, maintained, or transmitted by Live Well Wellness Center in connection with our healthcare services. These services may include naturopathic primary care, functional and integrative medicine, hormone replacement therapy, medical weight management, IV therapy, acupuncture, and other medical services offered by our providers.
This notice applies to our healthcare professionals, employees, and other individuals or entities acting on our behalf to the extent they are involved in handling protected health information.
2. Your Rights Regarding Your Medical Information
You have important rights concerning the medical information we maintain about you.
Right to Access Your Medical Records
You have the right to request access to, inspect, or obtain an electronic or paper copy of your medical records and other protected health information maintained in a designated record set.
We will process requests within the time limits established by applicable federal and Arizona law. HIPAA generally requires a response within 30 calendar days, subject to a permitted extension. Arizona law may require an earlier response depending on the circumstances and purpose of the request.
We may charge a reasonable, legally permitted fee for copies of medical records. In limited circumstances, we may deny access as permitted by law. If we deny your request, we will provide an explanation and information about any applicable review rights.
Right to Request Corrections
If you believe your medical information is incorrect or incomplete, you may request an amendment. We will review your request and respond within the time required by applicable law, generally within 60 days under HIPAA, subject to permitted extensions.
If we deny the requested amendment, we will provide a written explanation and information about your additional rights.
Right to Request Confidential Communications
You may request that we communicate with you through a particular method or at a specific location. For example, you may ask us to contact you only through your mobile telephone or send correspondence to a different mailing address. We will accommodate reasonable requests in accordance with applicable law.
Right to Request Restrictions
You may request restrictions on how we use or disclose your protected health information for treatment, payment, or healthcare operations. We are not generally required to agree to every requested restriction. If we agree, we will follow the restriction except where disclosure is otherwise permitted or required by law, including certain emergency circumstances.
If you pay for a healthcare service entirely out of pocket, you may request that information about that service not be disclosed to your health plan for payment or healthcare operations. We must honor that request unless disclosure is required by law.
Right to an Accounting of Disclosures
You may request an accounting of certain disclosures of your protected health information made during the six years preceding your request. The accounting generally excludes disclosures for treatment, payment, healthcare operations, and other categories excluded by HIPAA.
One accounting during a 12-month period will be provided without charge. We may charge a reasonable, cost-based fee for additional requests during the same period, as permitted by law.
Right to Receive a Copy of This Notice
You may request a paper copy of this Notice of Privacy Practices at any time, even if you previously agreed to receive it electronically. We will provide a copy promptly.
Right to Designate a Personal Representative
If someone has legal authority to make healthcare decisions on your behalf, that person may be permitted to exercise your privacy rights. We may require documentation verifying that authority. Applicable Arizona law may provide additional protections or limitations concerning minors and certain confidential healthcare services.
Right to File a Privacy Complaint
You may file a complaint if you believe your privacy rights have been violated. Complaints may be submitted to Live Well Wellness Center or to the U.S. Department of Health and Human Services Office for Civil Rights. We will not retaliate against you for filing a complaint or exercising your privacy rights.

Family Members, Caregivers, and Others Involved in Your Care
You may authorize or agree to the disclosure of relevant health information to family members, close friends, caregivers, or others involved in your care or payment for your care. 6. Uses and Disclosures Requiring Written Authorization
Certain uses and disclosures generally require your written authorization.
Marketing
We will obtain your written authorization before using or disclosing protected health information for marketing when required by HIPAA or other applicable law. Certain treatment-related communications, care coordination, and other legally excepted healthcare communications may not require separate authorization.
We will not treat participation in a patient consultation or submission of a medical intake form as blanket authorization to disclose protected health information to advertising companies.
Sale of Protected Health Information
We will not sell protected health information without the authorization required by applicable law.
Psychotherapy Notes
Most uses and disclosures of psychotherapy notes, as specifically defined under HIPAA, require separate written authorization.
Other Uses and Disclosures

Certain substance use disorder treatment records receive special confidentiality protections under federal law, including 42 CFR Part 2. If Live Well Wellness Center receives or maintains records subject to Part 2, we will handle them in accordance with applicable federal confidentiality requirements.
